3Dz Dance Studio – Kids Program Waiver & Release of Liability
IMPORTANT: All participants must complete this form before attending any classes at 3Dz Dance Studio. If the participant is under 18 years old, a parent or legal guardian must also sign. Admission to class or rehearsal will not be granted without a properly completed and signed waiver.

Kids Program Policy
– Kids’ programs are available for children aged 4 years and above.
– Programs are designed to foster creativity, physical development, and social interaction in a safe and enjoyable environment.
– A minimum of 3 enrolled students is required for any program to run. If fewer than 3 students register, the program will not commence.

Program Registration Policy
– All program fees must be paid in full before the start date.
– Late payments may result in the student being denied access to classes.

Make-Up Lessons
– Make-up classes are only available in other open classes and may not cover the same content as the missed class.
– All make-up lessons must be completed within the same term; unused make-ups will expire.

Refund Policy
– All payments for kids’ programs are non-refundable, regardless of attendance, withdrawal, or scheduling conflicts.
– This policy applies to all seasonal enrollments, class packages, and rehearsals.
– Please confirm your child’s availability before registering.

Email Marketing Consent
I would like to receive updates, promotions, newsletters, and special offers from 3Dz Dance Studio by email. I understand that I can unsubscribe at any time by clicking the unsubscribe link in emails or by contacting the studio directly.

Assumption of Risk
I acknowledge that participating in dance lessons and related activities at 3Dz Dance Studio involves potential risks, including but not limited to:
– Personal injury
– Illness or aggravation of existing conditions
– Property damage or loss
I freely accept and fully assume all such risks, dangers, and hazards, whether caused by myself, other participants, equipment, or studio conditions.

Release of Liability, Waiver of Claims & Indemnity Agreement
In consideration of being permitted to participate in lessons and activities at 3Dz Dance Studio, I agree to the following:
– To Waive Any and All Claims that I or my child may now or in the future have against 3Dz Dance Studio, its owners, directors, employees, contractors, and equipment suppliers (collectively, the “Releasees”).
– To Release the Releasees from any and all liability for loss, damage, injury, or expense that I, my child, or my next of kin may suffer, whether due to negligence, breach of contract, breach of duty of care, or otherwise.
– To Hold Harmless and Indemnify the Releasees from any liability for damage to property, personal injury, or costs incurred by a third party as a result of my child’s participation in studio activities.

I understand that:
– 3Dz Dance Studio staff will strive to provide a safe environment but cannot eliminate all risks.
– Staff may be unaware of participants’ health conditions or limitations.
– Equipment may malfunction or fail.

Instructors cannot supervise every student at all times.
I voluntarily accept these risks and allow my child to participate despite them.

Medical Responsibility
I acknowledge that it is my responsibility to ensure my child has adequate medical, dental, accident, and personal insurance coverage.

Media Release
I give permission to 3Dz Dance Studio to record and use photos, videos, or quotations of my child in promotional materials (print or digital, including social media).
I waive any right to compensation or future recourse related to these materials.

Binding Effect
By clicking the box below, I acknowledge that I am the parent or legal guardian of the participant named above, and that I am legally authorized to enter this agreement on their behalf. This waiver and release shall be binding upon myself, my child, and our heirs, executors, administrators, and assigns. If any portion of this agreement is deemed unenforceable, the remainder shall remain in full force and effect.

Acknowledgment of Understanding
By checking the box below, I confirm that I have read and understood this agreement in full. I acknowledge that by agreeing electronically, I am voluntarily waiving certain legal rights on behalf of myself and my child, including the right to sue 3Dz Dance Studio for negligence.
